Men are filling their testicles with liters of saline in a growing trend known as 'ballmaxxing.' While typical testicles are the size of a small egg or plum, an online Reddit community of nearly 10,000 men shows them inflating their scrotums to the size of grapefruits. Participants often photograph their genitals with needles inserted and taped into the scrotum, administering saline drips purchased online. This causes temporary swelling, often enveloping the penis and making it appear smaller.

Roots in Body Modification Kink

Rooted in body modification kink, ballmaxxing can be sexual or non-sexual but carries significant risks. Dr. Shirin Lakhani, a sexual health specialist, warns: 'Ballmaxxing or scrotal inflation with saline is considered quite dangerous. The scrotum area is very sensitive and contains delicate structures, including testes, blood vessels, and nerves, not designed to accommodate fluid distension.'

Why People Engage in Ballmaxxing

Sex expert Gigi Engle, author of Kink Curious, explains that ballmaxxing falls under body modification kink, altering the body for sexual arousal or aesthetic gratification. 'It's linked to “freakshow” type body modification celebrating extreme size. Things being larger than life is part of the erotic appeal,' she says. Participants enjoy the weight and sensation of inflated scrotums, with some describing a 'tingling' from skin stretching. There can also be a BDSM dynamic, where a dominant partner enjoys the large genitals while the submissive enjoys the sensation.

Non-sexual motivations include perceptions of masculinity. 'Having big balls is linked to being “more of a man,” associated with bravery and dominance,' Engle adds.

Distinction from 'Scrotox'

Ballmaxxing differs from 'scrotox,' where men receive Botox injections in the scrotum for smoother skin and reduced sweat. Ballmaxxing uses saline to achieve extreme enlargement. Dr. Lakhani notes increased interest, especially among men over 40, as stigma around male cosmetic procedures decreases.

Health Risks

Despite its niche appeal, ballmaxxing is extremely risky. Dr. Lakhani lists dangers including bacterial infection, abscess formation, cellulitis, tissue or nerve damage, microtears, bruising, and impaired blood flow to testicles. Symptoms include burning sensation, pain, swelling, and inflammation. Erectile dysfunction can result from disrupted blood flow, and more severe complications include infertility, blood clots, and gangrene, which requires surgical removal of dead tissue.

Gigi Engle advises that if attempted, saline should be administered by a professional. The trend, while growing, remains dangerous and not recommended by medical experts.
