Recognizing what is normal for you and seeking medical advice for persistent or unusual symptoms is crucial, as many causes are treatable.

Erectile Dysfunction as a Warning Sign

Dr Ashrafi explains that erectile dysfunction is often a symptom of an underlying issue rather than a standalone condition. He says, 'An erection relies on healthy blood vessels, nerves, hormones and psychological wellbeing all working together. If something is disrupting one of those systems, the penis can sometimes be one of the first places it becomes noticeable.'

Because the blood vessels supplying the penis are smaller than those supplying the heart, erectile changes can be an early sign of high blood pressure, as they may narrow or lose elasticity. Dr Ashrafi notes that 'some men may notice changes to their erections before they experience any other symptoms,' and persistent changes should not be attributed solely to age.

Diabetes, Cholesterol, and Hormones

Diabetes can affect the blood vessels and nerves involved in erections. If accompanied by increased thirst, frequent urination, blurred vision, or unexplained fatigue, Dr Ashrafi advises consulting a GP to 'rule out' the condition. High cholesterol may also manifest as erectile dysfunction; Dr Ashrafi warns that 'for some men, changes in erectile function can be an early clue that it's time to have their cholesterol checked.'

Low testosterone can impact sexual desire and function. If reduced sex drive occurs with persistent tiredness, low mood, reduced muscle strength, or fewer spontaneous erections, hormones could be a factor. Additionally, poor sleep quality, including loud snoring or excessive daytime tiredness, may contribute to erectile changes, potentially indicating a sleep disorder.

Skin Changes, Curvature, and Pain

New marks, lumps, or rashes on the penis can be linked to STIs like HPV or herpes, but some, such as pearly penile papules, are harmless. Dr Ashrafi advises against self-diagnosis and recommends speaking to a healthcare professional if a change is new, painful, persistent, spreading, or accompanied by discharge. A new or worsening curve, especially with pain or a lump, may indicate Peyronie's disease, where scar tissue develops; treatment options are available.

Pain during sex or erections can stem from infections, inflammation, or Peyronie's disease. Dr Ashrafi adds, 'If the pain is accompanied by symptoms such as sores, blisters, unusual discharge or pain when urinating, it's particularly important to consider whether a sexually transmitted infection could be responsible.' Difficulty retracting the foreskin, or phimosis, may require medical attention if it causes pain or interferes with urination; treatments range from steroids to surgery.

Swelling, Smell, and Sensation

Redness, soreness, swelling, itching, discharge, or an unpleasant smell can point to an STI or balanitis, an inflammation of the glans. Dr Ashrafi says, 'Recurrent balanitis can sometimes also be associated with underlying conditions such as diabetes, so repeated episodes are something you should discuss with your GP rather than continually treating yourself.'

Persistent numbness or reduced sensation may be unrelated to the penis itself. 'Conditions such as diabetes can affect the nerves, while certain nutritional deficiencies, including vitamin B12 deficiency, can also cause neurological symptoms,' says Dr Ashrafi. He recommends consulting a doctor for long-term sensation issues, especially if numbness or tingling occurs elsewhere in the body.
